About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at SunCrest OMNI Home Health in Pensacola, a part of LHC Group. As a Full-Time LPN, you will play a vital role in providing high-quality patient care and performing technical skilled care in the patient's home.

Responsibilities

  • Provide quality patient care and perform technical skilled care in the patient's home, following all applicable infection control procedures.
  • Continually observe and report the patient's nursing needs to supervising RN.
  • Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs.
  • Document patient visits thoroughly and completely, maintaining an accurate and current medication listing.
  • Actively participate in the performance improvement plan, working to reduce patient hospitalizations and improve outcomes.
  • Attend and actively participate in all meetings, including case conferences, and maintain a positive attitude towards patient care and agency operations.

Requirements

  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
