Job Summary:

We are looking for a dedicated Physical Therapist to provide high-quality care to patients in their homes or alternate community settings. As a Physical Therapist with VNA Health Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Assess and evaluate patients for physical therapy needs and develop individualized plans of action in collaboration with physicians.
  • Provide physical therapy evaluation and treatment to patients in their homes or alternate community settings.
  • Document all changes in patient condition and update care plans and outcomes accordingly.
  • Identify counseling needs and plan appropriate teaching strategies.
  • Monitor quality and appropriateness of services provided.
Requirements:
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills.
  • Strong communication and interpersonal skills.
  • Valid NJ driver's license.
