Nurse Practitioner or Physician Assistant

Optum Home & Community Care is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the seamless delivery of care to our patients, working closely with healthcare providers, families, and caregivers to develop and implement personalized care plans.
Key Responsibilities:- Deliver high-quality, cost-effective care to assigned members, managing both medical and behavioral conditions effectively.
- Perform comprehensive assessments, documenting findings in a concise and compliant manner, adhering to CMS regulations.
- Ensure accurate and timely documentation of diagnoses, medications, and treatment plans.
- Facilitate agreement and implementation of the member's plan of care, engaging facility staff, families, and primary and specialty care physicians.
- Monitor the effectiveness and efficiency of the plan, making revisions as needed.
- Utilize practice guidelines and protocols established by CCM to ensure high-quality care.
- Certified Nurse Practitioner through a national board.
- Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
- Active and unrestricted license in the state of residence.
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Access to reliable transportation for travel to client and/or patient sites.
- Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions.
- Availability to work 24 hours per month, with expectations of 16 hours/month during off-hours.
- 1+ years of hands-on post-grad experience within Long Term Care.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
- Proficient computer skills, including documentation of medical information with written and electronic medical records.
- Ability to develop and maintain positive customer relationships.
