Physical Therapist Assistant

Job Summary:
We are seeking a skilled Physical Therapy Assistant to join our team in a sub-acute/skilled nursing facility setting. As a Physical Therapy Assistant, you will work collaboratively with other rehab therapists and nursing staff to provide comprehensive and compassionate therapy services to our residents.
Responsibilities:
- Provide therapy services to residents in a sub-acute/skilled nursing facility setting
- Work collaboratively with other rehab therapists and nursing staff to achieve optimal patient outcomes
- Develop and implement individualized therapy plans to meet the unique needs of each resident
- Document resident progress and communicate with other healthcare professionals as needed
- Participate in ongoing education and training to stay current with best practices in physical therapy
Requirements:
- Current Physical Therapy Assistant license or eligibility to practice in the state employed
- Strong computer skills with Microsoft Outlook, Word, Excel, and experience with electronic documentation
- Good attention to detail and timeliness with documentation
- Ability to work well in a team setting
