Job Summary

The Clinical Pharmacist will provide high-quality pharmaceutical care to patients, ensuring safe and effective medication use. This role requires strong communication and collaboration skills, as well as the ability to work effectively in a multidisciplinary team.

Key Responsibilities
  • Develop and implement medication therapy plans to improve patient outcomes
  • Provide accurate and timely medication information to healthcare professionals and patients
  • Supervise and mentor pharmacy staff to ensure accurate and quality pharmaceutical services
  • Participate in quality improvement initiatives to enhance patient care and safety
  • Collaborate with healthcare teams to develop and implement evidence-based clinical practice guidelines
Requirements
  • Current license as a Pharmacist by state in which one practices
  • Controlled substance license
  • Basic Life Support Certification (BLS) for healthcare providers
  • Advanced Cardiovascular Life Support (ACLS) Certification
  • Pediatric Advanced Life Support (PALS) Certification
Preferred Qualifications
  • Doctor of Pharmacy degree from an accredited college of pharmacy
  • Three years experience in an institutional pharmacy environment
Work Environment

This position will be based at American Fork Hospital, working closely with the multidisciplinary team to provide high-quality patient care.
