About the Role

As a Healthcare Professional - Hospice Aide, you will play a vital part in providing compassionate care to patients in their homes. With a strong focus on delivering high-quality patient-centered care, our team works collaboratively to ensure that each patient receives the support they need to maintain their independence and dignity.

We are seeking an experienced Home Health Caregiver to join our dynamic team. This role is ideal for individuals who are passionate about delivering exceptional patient care and possess excellent communication skills.

Key Responsibilities

- Provide personalized care to patients in their homes, including assisting with daily living activities, medication reminders, and light housekeeping tasks
- Develop and implement individualized care plans tailored to each patient's unique needs and preferences
- Collaborate with interdisciplinary teams to ensure seamless coordination of care and optimal patient outcomes
- Maintain accurate and detailed records of patient interactions, progress, and concerns

Requirements

- Minimum 6 months experience in home health care or a related field
- Strong understanding of patient-centered care principles and practices
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a collaborative team
