Hospice Licensed Vocational Nurse (LVN) Job Description

We are seeking a dedicated Hospice Licensed Vocational Nurse (LVN) to join our interdisciplinary team and contribute to our mission of providing comfort, dignity, and support to individuals facing terminal illness.

Key Responsibilities:
  • Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life
  • Work as part of an interdisciplinary team comprised of a physician, RN, CNA, HHA, social worker, chaplain, and volunteers to carry out the patient's plan of care
  • Perform nursing services which are planned, delegated, and supervised by the registered nurse
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Take a holistic approach to care that engages the patient's mind, body, spirit, and family
Requirements:
  • Graduate of an accredited vocational nursing program and active LVN license in California
  • One year of hospice LVN experience
  • CPR certification
  • Valid California driver's license, auto insurance, and reliable transportation
  • Knowledge of state and federal home health and hospice regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
