Position Overview

Role: RN MDS Coordinator

Salary: $90,000 annually

Employment Type: Full-time, on-site, with a schedule from 7 AM to 3 PM, Monday through Friday

About the Role: HCP Talent is collaborating with a prominent Long-Term Care and Rehabilitation facility seeking a dedicated RN MDS Coordinator.

Key Responsibilities:

  • Evaluate and ensure the quality of care provided to long-term residents.
  • Conduct thorough assessments and interviews for new residents.
  • Assist in formulating detailed care plans with both immediate and future goals.
  • Lead discussions with multidisciplinary teams, caregivers, and family members through effective meetings.
  • Engage in Nursing management team discussions and collaborate with Nursing staff at all levels.
  • Facilitate informative in-service training sessions regarding MDS, CAAs, and Triggers.
  • Consistently follow the MDS/Care Planning timeline.

Qualifications:

  • Current and valid RN license in New York.
  • Minimum of 1 year of experience in a Skilled Nursing Facility.
  • Familiarity with MDS, Medicare, and the care-planning process.
