The estimated salary range for this position is $70,000 - $95,000 annually, based on qualifications and location.
About the Position:
The ideal candidate will have previous experience in nursing or a related field, with a strong focus on patient care and safety. They will be responsible for assessing patient needs, developing individualized care plans, and coordinating with other healthcare professionals to ensure comprehensive care.
Key Responsibilities:
- Conduct thorough assessments of patient needs and develop personalized care plans.
- Collaborate with healthcare teams to coordinate patient care and ensure continuity of services.
- Provide education and support to patients and their families regarding health maintenance and disease prevention.
- Document patient interactions and outcomes in accordance with established policies and procedures.
Requirements:
- Degree in Nursing or related field.
- Current licensure as a Registered Nurse.
- At least 2 years of experience in a clinical setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
