Clinical Social Worker

We are seeking a skilled Clinical Social Worker to join our team at Corewell Health. As a key member of our interdisciplinary team, you will work collaboratively with patients and families to address complex psychosocial needs.
Key Responsibilities- Conduct comprehensive psychosocial assessments to identify patient needs and develop effective care plans.
- Collaborate with healthcare teams to coordinate care and ensure seamless transitions.
- Provide consultation and resources to patients and families to support their well-being.
- Develop and maintain knowledge of community resources to support patient needs.
- Participate in departmental committees and projects to advance patient care.
- Master's degree in social work.
- State of Michigan Social Work License (LMSW) or equivalent.
- Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning.
