At Signature Health, we are dedicated to providing integrated healthcare services to our community, specializing in patients with mental illness and/or addiction. We are seeking a highly skilled Clinical Liaison, Geriatric Services to join our team.

Key Responsibilities
  • Coordinate communication with partner facilities, families, and patients regarding program services and support.
  • Advocate for patients by providing outreach, education, assessment, and treatment support to facilitate access to care and collaboration between patients, families, and care teams.
  • Travel to patient homes and care facilities throughout our service area to conduct face-to-face diagnostic assessments and provide education and support.
  • Develop and maintain relationships with partner facility clinical and administrative staff to ensure smooth communication and collaboration.
  • Provide advocacy, education, and support to individual patients and families regarding aging-specific matters and community resources.
  • Facilitate community education efforts to congregate living facilities and other community organizations.
  • Engage stakeholders in the mental health and aging networks to advance program growth goals.
Requirements
  • Master's Degree required.
  • Valid unencumbered independent license (LISW, LPCC, or LIMFT) in the state of Ohio required.
  • 1-2+ years of assessment or counseling skills experience preferred.
  • 1-2+ years of experience with the aging population preferred.
  • Proficient in Microsoft Office and Electronic Health Records.
Working Conditions

Work is normally performed in a typical interior/office/clinical work environment. Additional work may be completed in home-based environments. While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
