Patient Care Navigator

We are seeking a highly skilled and compassionate Patient Care Navigator to join our team at HME Specialists LLC.
About the RoleThis is a full-time position that involves working directly with patients in select hospitals, delivering essential medical equipment and providing education on its effective use.
You will also build strong relationships with Case Managers, ensuring a seamless discharge process for patients with respiratory needs.
Key Responsibilities- Deliver medical equipment to patients in their rooms and provide personalized instruction on its proper use.
- Facilitate communication between patients, caregivers, and healthcare professionals to ensure a smooth transition of care.
- Manage phone calls and coordinate with Customer Service to process new orders efficiently.
- Experience working in fast-paced environments and handling multiple tasks simultaneously.
- Excellent communication and interpersonal skills to effectively interact with patients, caregivers, and healthcare professionals.
- Ability to empathize with patients and their families during challenging times.
- Familiarity with relevant software and tools to manage orders and coordinate with customer service.
A competitive salary of $55,000 per year, paid bi-weekly, reflecting the industry standards and cost of living in Las Cruces. Our comprehensive benefits package includes:
