We are seeking a compassionate and skilled Medical Social Worker to join our team at Trinity Hospice of Aiken, a part of LHC Group. As a Medical Social Worker, you will play a vital role in providing emotional and social support to patients and their families, helping them navigate the complexities of hospice care.
Responsibilities- Assess patients' social and emotional needs and develop individualized plans to address these needs.
- Collaborate with the healthcare team to ensure comprehensive care and support for patients and their families.
- Provide education and resources to patients and families on coping mechanisms, grief support, and community resources.
- Contribute to the development of a comprehensive plan of care that addresses the physical, emotional, and social needs of patients.
- Master's degree in Social Work from a Council of Social Work Education-accredited program.
- One year of experience in a healthcare setting, preferably in hospice care.
- Current CPR certification and a valid driver's license.
