Job Summary

We seek a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessor. This role involves conducting adult and geriatric assessments, medical history reviews, diagnosis, treatment, health education, and documentation according to Matrix guidelines and protocols.

The successful candidate will work collaboratively with physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel to provide comprehensive patient care.

Duties and Responsibilities

  • Conduct in-depth assessments of patients' medical histories, current health status, and risk factors.
  • Develop and implement personalized treatment plans, ensuring accurate documentation and follow-up care.
  • Collaborate with healthcare professionals to ensure seamless patient care, including physician referrals and case management.
  • Provide health education and guidance to patients and their families, promoting informed decision-making and self-care.
  • Travel to various locations, including patients' homes, skilled facilities, and community environments, to deliver care and conduct assessments.

Requirements

  • Masters Degree in Nursing required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain.
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Valid BLS, ACLS, or CPR certification.
  • Minimum 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health records software.
  • Excellent verbal and written communication skills for interacting with patients, clients, and colleagues.
  • Able to adapt to changing circumstances and prioritize tasks efficiently.

Competitive hourly rate: $105.00 or $110.00 based on county per completed assessment.
