Epic Health Partners is seeking a highly skilled Mental Health Program Coordinator to join our team. As a key member of our clinical staff, you will be responsible for providing oversight of clinical service delivery for our Mental Health service lines.
Key Responsibilities- Service Delivery Oversight: Provide general oversight of clinical service delivery for Mental Health service lines, including face-to-face sessions, treatment plan development and implementation, and discharge planning.
- Progress Note Review: Review progress notes to ensure accurate and timely documentation.
- New Referral Management: Manage the process for new referrals, ensuring all required documentation is completed accurately and in a timely manner.
- ISP/Quarterly Reviews: Conduct ISP/Quarterly reviews to ensure compliance with contract, accreditation, and licensure expectations and standards.
- Discharge Summaries: Complete discharge summaries to ensure a smooth transition of care.
- Caseload Management: Maintain a minimum caseload providing individual, group counseling, assessment, and/or crisis intervention to adults and families who are experiencing psychological, or adjustment difficulties or problems related to substance use.
- Clinical Metrics: Complete clinical metrics, including SAR – Approvals and Denials.
- Assessment and Treatment Planning: Conduct and/or review and cosign all assessments, diagnoses, and treatment plans within 24 hours of development or prior to the initiation of treatment.
- Service Authorization Requests: Submit Service Authorization Requests (SAR) to identify MCOs to express clinical and medical necessity for requested services.
- Peer-to-Peer Reviews: Participate in peer-to-peer reviews with identified MCO, if required.
- Collaborative Sessions: Conduct collaborative sessions with individuals and QMHP's to facilitate the completion of treatment plans and quarterly reviews.
- Record Review: Assist in oversight of the quantitative and qualitative record review process for individual files.
- Policy and Procedure Updates: Inform staff of changes in policy and procedures as needed.
- Temporary Housing Options: Manage and monitor temporary housing options and finances associated with those resources.
- Partnerships and Collaboration: Demonstrate effective knowledge of Epic Health Partners' services and build partnerships with potential stakeholders.
- Meetings and Communication: Attend, lead, and participate in meetings as requested, and maintain confidentiality and security of Protected Health Information (PHI) as outlined in the Confidentiality Agreement.
- Travel and Transportation: Travel to other locations as required and provides safe transportation according to EPIC's policies and guidelines as needed to perform job duties.
To be successful in this role, you will need to have a strong understanding of clinical service delivery, excellent communication and interpersonal skills, and the ability to work effectively in a team environment.
