About the Role

We are seeking a skilled Registered Nurse to join our team as a Case Manager. In this role, you will provide direct patient care, coordinate home care services, and document care for agency clients.

Key Responsibilities
  • Ensure quality and safe delivery of home health services, including case management and OASIS evaluations.
  • Develop and implement plans of care based on client assessments and OASIS evaluations.
  • Coordinate with LPNs, provide guidance and education on client care, and work with the Clinical Supervisor and Patient Care Coordinator to manage patients effectively.
  • Supervise paraprofessional staff, including home health aides, and communicate information to the home health aide department.
Requirements
  • Current New York State registered professional nurse license.
  • BSN degree preferred.
  • Valid driver's license and auto insurance.
  • Self-directed and organized with the ability to work independently.
  • Previous home care or case management experience preferred.
