Job Title: Registered Nurse, Case Manager

Nascentia Health is seeking a skilled Registered Nurse to join our team as a Case Manager. As a key member of our home care team, you will be responsible for providing high-quality patient care and coordination of services to ensure the best possible outcomes for our clients.

Responsibilities:
  • Provide direct patient care and coordinate overall home care services in accordance with Quality Standards.
  • Develop and implement individualized plans of care based on client assessments and OASIS evaluations.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination of services.
  • Document patient care and interactions in a clear and concise manner, meeting organizational standards.
  • Supervise and educate LPNs, home health aides, and other paraprofessional staff as needed.
  • Comply with all Nascentia Health policies and procedures, maintaining patient and employee confidentiality at all times.
Requirements:
  • Current New York State registered professional nurse license (degree from an accredited school of nursing).
  • BSN degree preferred.
  • Valid driver's license and auto insurance, with a vehicle available for work.
  • Self-directed and organized, with the ability to work independently in a fast-paced environment.
  • Previous home care or case management experience a plus.
  • Proficient computer skills, including knowledge of electronic medical records software.
