Key Responsibilities:
Deliver patient care in a safe and timely manner, utilizing recognized nursing skills and best practices as defined by Adoration's quality assurance team.
Actively participate in weekly team meetings while adhering to current COVID safety protocols to coordinate appropriate patient care.
Implement nursing interventions that promote patient rehabilitation and achieve quality, outcome-driven results.
Provide compassionate nursing care while collaborating with other members of the interdisciplinary treatment team as necessary.
Adjust the nursing care plan as needed to ensure patients reach their maximum potential.
Administer medications as prescribed by the patient's Primary Care Physician, including monitoring and providing instruction.
Conduct patient assessments and collaborate with interdisciplinary team members to develop the patient's care plan.
Supervise Licensed Practical Nurses and Home Health Aide staff as appropriate.
Communicate changes in the client's medical needs to the physician and nursing supervisor through proper documentation and communication methods.
Complete timely documentation of all patient care, including progress notes, summaries, and discharges in accordance with state and federal regulations.
Assist in creating and implementing individualized treatment plans that facilitate rapid recovery at home.
Adhere to safe nursing practices and promptly report patient events to the nursing supervisor.
Help maintain and update nursing, pharmacy, infection control, and health and safety policies and procedures as requested.
Be a self-motivated individual comfortable working within a supportive clinical team.
Familiarity with electronic medical records and documentation software (WellSky) is preferred.
Other duties as assigned.
Qualifications:
Registered Nursing Degree (Associate or Bachelor) from an accredited nursing college with current unrestricted registration and license in the applicable state required.
Minimum one year of nursing experience in a patient care setting required; experience in home health, geriatrics, or similar settings preferred.
Valid driver's license, acceptable driving record, and proof of car insurance in accordance with company policy.
Current CPR certification.
