Job Title: Home Health Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health team. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.

Responsibilities:
  • Assess and evaluate patients' needs, developing individualized care plans to meet their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress, updating care plans and outcomes as needed.
  • Identify teaching needs and develop strategies to educate patients and families on health management and self-care.
Qualifications:
  • Active New Jersey RN License
  • Bachelor's degree in Nursing preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
