Job Title: Home Health Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team in providing high-quality home health care to patients in Essex County, NJ.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide skilled nursing care to patients in their homes. The ideal candidate will have a strong background in home health care, excellent communication skills, and a passion for delivering exceptional patient care.

Responsibilities:
  • Assess and evaluate patient needs, developing individualized care plans to meet their unique requirements.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Document patient progress, updating care plans and outcomes as needed.
  • Identify teaching needs and develop strategies to educate patients and families on disease management and self-care.
Requirements:
  • Current RN licensure in NJ.
  • 1+ year of supervised clinical experience as a Registered Nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills and ability to navigate electronic medical records.
  • Valid driver's license and reliable transportation.
