About the Role

We are seeking a skilled Home Health Nurse Specialist to join our team at PeaceHealth. This is a full-time position with a 1.00 FTE, day schedule.

Compensation

The estimated salary for this role is $63,449 - $97,608 per annum, based on the location and requirements of the job.

Job Description

The Home Health Nurse Specialist will provide direct skilled nursing care and coordination/management of patients admitted to home care/clinic services. The ideal candidate will utilize considerable judgment and skill in making patient care decisions, applying the professional healthcare process of assessment, problem identification/planning/intervention, and clinical/goal outcome evaluation.

Essential Functions
  • Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.
  • Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
  • Perform physical assessments, develop, coordinate, modify, and manage written individualized plans of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely home health outcomes.
  • Develop and clinically manage the Home Health Aide's care plan according to the patient's evolving personal care needs.
  • Coordinate care, collaborate, and communicate with other members of the interdisciplinary team to ensure appropriate and smooth flow of care/services through the care continuum.
  • Perform discharge planning appropriately towards patient-specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
  • Provide accurate, complete, and timely clinical documentation in compliance with regulatory, quality, and Agency guidelines.
  • Assist with the plan and implementation of family bereavement services for patient death.
Qualifications
  • Accredited School of Nursing Required: Nursing and Bachelor's Degree Preferred: Bachelor of Science in Nursing.
  • 2 years Required: Acute clinical or home health/hospice nursing experience; 1 year Required: Home health experience preferred or will orient to point of competency.
  • Demonstrated knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient's needs.
Credentials
  • Required: Registered Nurse in state of practice.
  • Required: Driver's License and proof of auto insurance.
  • Required Upon Hire: Basic Life Support.
