Mental Health Outreach Coordinator

As a Mental Health Outreach Coordinator, you will play a vital role in our Certified Community Behavioral Health Clinic (CCBHC) in Riverhead. You will work as part of a multidisciplinary team to provide outreach support and coordination of care for clients.
About the Role- You will be responsible for facilitating client engagement in services and providing additional support to enable them to participate in treatment and live successfully in the community.
- You will coach and support clients in areas such as self-advocacy, aftercare, wellness management, medication adherence, smoking cessation, entitlements, and daily living activities.
- You will accompany clients to medical appointments and other engagements to facilitate their participation and follow-through.
- Provide concrete service delivery to clients, including benefits and enrollments, tailored to individual needs.
- Maintain accurate and organized records, statistics, and reports in accordance with policy and procedure.
- Coordinate care planning with other providers to ensure goal-directed, collaborative care, including care transitions.
- Act as a resource for team members on psychosocial and substance abuse issues and resources.
- Offer telephonic and face-to-face outreach, engagement, and service planning during clients' stays at the center and post-discharge.
- Link clients with community services, including medical, behavioral, residential, entitlement, and other necessary services as per interdisciplinary care plans.
- Monitor overall service delivery to clients to ensure coordination and continuity; advocate with service providers as needed.
- Provide crisis intervention and follow-up.
- Complete training and maintain proficiency in de-escalation and crisis intervention techniques.
- Perform all other duties assigned.
- A high school diploma or equivalent is required.
- Prior experience as a Peer Specialist in a related setting is preferred.
- The successful candidate will possess relevant life experience, including personal experience living with a disability and/or social/emotional challenge.
- Familiarity with New York State entitlement programs, housing, health, and/or mental health/substance abuse service delivery systems is desirable.
- Excellent interpersonal and communication skills are essential.
- Proficiency in Microsoft Office and Electronic Health Records is required.
- A valid and clean New York State Driver's License is mandatory.
- The ability and willingness to travel to different locations using your vehicle in the mobile response team are necessary.
- A generous compensation package includes a yearly salary of $39,000.
