Care Coordinator for Vulnerable Populations

Role Summary
We are seeking a full-time Care Coordinator to join our Certified Community Behavioral Health Clinic (CCBHC) in Riverhead. As a key member of our multidisciplinary team, you will play a vital role in coordinating care and supporting clients in their journey towards recovery.
About the Role
- You will provide outreach and support to clients, facilitating engagement in services and promoting successful living in the community.
- Using your expertise, you will offer coaching and recovery support, addressing issues such as self-advocacy, linkage to aftercare, wellness self-management, medication adherence, and daily living activities.
- Accompanying clients to medical and other appointments will be an essential part of your role, ensuring timely follow-through and care coordination.
- You will deliver concrete services, including benefits enrollment and assistance with daily living tasks, tailored to individual client needs.
- Maintaining accurate records and statistics will be crucial in evaluating service delivery and informing care planning.
- You will collaborate with healthcare providers and community resources to ensure goal-directed, coordinated care, including transitions between settings.
- Your expertise will be invaluable to the team, acting as a resource on psychosocial and substance abuse issues and available community resources.
- Telephonic and face-to-face outreach will be integral to your role, ensuring consistent support during clients' stays and post-discharge periods.
- Linkages to community services, including medical, behavioral, residential, entitlements, and other necessary services, will be facilitated through interdisciplinary care plans.
- Crisis intervention and follow-up will also be essential components of your work.
- Training and proficiency in de-escalation and crisis intervention techniques will be expected.
Requirements
- A High School Diploma or equivalent is required, with a Peer Specialist Certification or eligibility to obtain certification within one year of employment.
- At least one year of experience as a peer specialist in a related setting is preferred.
- Relevant life experience, including personal experience living with a disability and/or social/emotional challenge, is essential.
- Familiarity with New York State entitlements, housing, health, and/or mental health/substance abuse service delivery systems is desirable.
- Excellent interpersonal, verbal, and written communication skills are required.
- Proficiency in Microsoft Office and Electronic Health Records is expected.
- A valid New York State driver's license and ability to travel for work-related purposes are necessary.

Compensation Details
The annual salary for this position is $39,000, offering a competitive compensation package and opportunities for growth and development.
