Key Responsibilities
  • Primary Point of Contact: Serve as the primary point of contact between patients, families, home care agencies, and healthcare providers.
  • Patient Assessment: Assess patients' medical needs, home environment, and suitability for home care services.
  • Clinical Information Evaluation: Evaluate patients' clinical information, including medical history, medications, and current health status.
  • Transition of Care: Coordinate the transition of care from hospital to home, including arranging for necessary medical equipment, supplies, and home modifications.
  • Care Plan Development: Collaborate with physicians, hospital staff, and home care teams to develop and implement personalized care plans.
  • Patient Education: Educate patients and their families about the home care process, care plan, medications, and any specific medical instructions.
  • Guidance and Support: Provide guidance on managing chronic conditions, wound care, medication administration, and other necessary skills.
  • Referrals and Documentation: Facilitate referrals to appropriate home care services and ensure that all necessary documentation and authorizations are completed.
  • Compliance and Quality: Ensure compliance with healthcare regulations, standards, and best practices.
  • Complex Case Management: Manage complex cases, including coordinating with multiple healthcare providers and services.
  • Continuity of Care: Ensure continuity of care and address any issues or concerns that may arise during the home care process.
  • Record Keeping: Maintain accurate and up-to-date records of patient assessments, care plans, communications, and services provided.
  • Progress Reporting: Report to healthcare providers and home care agencies on the patient's progress and any changes in their condition.
Requirements
  • Education: Graduate of an accredited school of nursing.
  • License: Currently registered in New York State as a Registered Professional Nurse.
  • Experience: Home care experience is a plus.
  • Reliable Transportation: Must have a reliable vehicle to travel to patients.
