**Job Overview**

We are seeking a skilled Home Health Physical Therapist to join our team at Jet Health in Littleton, Colorado. This is an excellent opportunity for a dedicated and professional Physical Therapist to provide coordinated care to patients of all age groups.

About the Job:

This full-time position involves planning and implementing individualized physical therapy treatment programs based on medical referrals and evaluations. You will work closely with other healthcare professionals to ensure comprehensive treatment and coordinate care with patients' families to promote their well-being.

Responsibilities:

  • Develop and implement individualized physical therapy treatment plans.
  • Evaluate and document patient progress, adjusting treatments as necessary.
  • Coordinate care with other healthcare professionals to ensure effective treatment.
  • Educate patients and their families on treatment plans, home exercises, and preventative care.
  • Maintain accurate and timely patient records in compliance with company and regulatory standards.

Requirements:

  • Bachelor's degree in Physical Therapy from an American Physical Therapy Association-approved program.
  • Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC.
  • At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.
  • Proficiency in therapy skills as defined by accepted standards of practice. Strong interpersonal skills. Current CPR certification. Knowledge of durable medical equipment.
