About the Role: Jet Health is seeking a dedicated and professional Physical Therapist to join our team serving patients in the Littleton, Colorado area.

Job Summary:

We are looking for an experienced Physical Therapist to provide coordinated care to patients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being. The ideal candidate will have strong interpersonal skills, current CPR certification, and knowledge of durable medical equipment.

Key Responsibilities:
  • Develop and Implement Treatment Plans: Create personalized physical therapy treatment plans based on medical referrals and evaluations.
  • Evaluate and Document Progress: Regularly assess patient progress and document changes to adjust treatments as necessary.
  • Care Coordination: Collaborate with healthcare professionals to ensure comprehensive treatment and effective coordination of care.
  • Patient Education: Educate patients and their families on treatment plans, home exercises, and preventative care.
  • Record Keeping: Maintain accurate and timely patient records in compliance with company and regulatory standards.
  • Durable Medical Equipment: Utilize necessary equipment and provide training to patients as needed.
  • Patient Safety: Ensure patient safety and comfort during therapy sessions.
Requirements:

To be successful in this role, you must have:

  • Education: Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
  • Licensure: Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC).
  • Experience: At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.
  • Skills: Proficiency in therapy skills as defined by accepted standards of practice, strong interpersonal skills, and current CPR certification.
