Home Health Occupational Therapist Opportunity

BAYADA Home Health Care is seeking a highly skilled and experienced occupational therapist to join our team in Littleton, Colorado. As an occupational therapist with BAYADA, you will have the opportunity to make a meaningful difference in the lives of our clients while advancing your career.
About the RoleThis full-time or part-time occupational therapist position involves working with clients in their homes, providing rehabilitative care and skilled nursing services. The ideal candidate will have prior experience in home health care, preferably with Medicare Part A patients. In-patient facility experience or knowledge of Medicare Part B services is also desirable.
The coverage area for this position includes territories south of I-70 and both east and west of I-25. You will be responsible for making home visits, assessing patients' functional status, developing individualized intervention plans, and educating clients and their families on rehabilitative care and activities necessary for independent living.
Responsibilities- Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
- Evaluate the home environment for hazards or barriers to more independent living and identify equipment needs and environmental adaptations.
- Continually assess and revise the occupational therapy care plan, participating in the multidisciplinary care plan as needed.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary for health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
- A current CO Occupational Therapist license.
- Prior home care experience is preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
