We are seeking a skilled Licensed Practical Nurse (LPN) to join our Hospice team. As a LPN, you will play a vital role in providing high-quality care to our patients and their families.
Responsibilities- Provide direct patient care, including pain and symptom management
- Implement nursing practice based on comprehensive assessments and care plans
- Document patient services accurately and in a timely manner
- Assess and educate patients and their families/caregivers
- Participate in coordination of hospice services
- Manage patient pain and symptoms effectively
- Participate in professional growth and development opportunities
- Ensure compliance with state and federal regulations
- Graduate of an accredited practical or vocational nursing program with current licensure
- 1 year of nursing experience, with 3+ years preferred
- Experience in community health, hospice, or med/surg preferred
- Knowledge of hospice philosophy and commitment to clinical excellence
