  • Collaborate with the interdisciplinary team to create and implement individualized care plans that meet the physical, emotional, and spiritual needs of patients and their loved ones.
  • Provide leadership and support to the hospice staff, promoting a culture of compassion and excellence.
  • Conduct thorough assessments of patients and their loved ones, identifying their unique needs and developing effective care plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Manage patient care and coordinate services, ensuring timely and efficient delivery of care.
  • Participate in ongoing education and training to enhance your skills and knowledge in hospice care.
Requirements
  • Registered Nurse licensed in the state of Georgia.
  • CHPN (Certified Hospice and Palliative Nurse) certification preferred.
  • CPR Certification.
  • Minimum of one year general nursing experience, preferably in hospice, home health, medical, surgical, or critical care.
  • Valid driver's license and reliable transportation.
