Job Title: Registered Nurse Case Manager

At BrightSpring Health Services, we are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Spring Valley Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients and their families.

About the Role:

We are looking for a dedicated and experienced Registered Nurse to provide comprehensive care to our patients. Your primary responsibilities will include:

  • Conducting initial and ongoing assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborating with the interdisciplinary team to ensure seamless care coordination and communication.
  • Administering medication and treatment as prescribed by the physician, while also providing education and support to patients and families.
  • Maintaining accurate and detailed clinical notes to ensure continuity of care.
  • Participating in interdisciplinary group meetings to discuss patient progress and develop strategies for optimal care.
Requirements:

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited institution.
  • Current licensure to practice as a Registered Nurse in the state of operation.
  • At least one year of experience in nursing, with a preference for experience in hospice or a similar setting.
  • Knowledge of the hospice philosophy of care and a commitment to clinical and documentation excellence.
