Job Overview

We are seeking a skilled and compassionate Registered Nurse to join our team as a Senior RN in our Elderplace program. As a core member of our interdisciplinary team, you will play a critical role in delivering high-quality care to at-risk elderly patients with complex medical and psychosocial issues.

The ideal candidate will have experience in home health care, preferably with a focus on geriatric patients, and possess strong clinical skills, excellent communication abilities, and a passion for patient-centered care.

Responsibilities
  • Provide comprehensive care management, care coordination, and skilled care services to frail elder patients
  • Develop and implement personalized care plans that address the unique needs and goals of each patient
  • Collaborate with the interdisciplinary team to ensure seamless coordination of care and services
Requirements
  • Bachelor's Degree in Nursing or related field
  • Oregon Registered Nurse License (upon hire)
  • National Provider BLS - American Heart Association (within 30 days of hire)
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
  • 1 year home health RN experience preferred
