Duke University Health System seeks a skilled Nurse Care Specialist to join our team. As a Registered Nurse in home health care, you will be responsible for providing high-quality patient care in the comfort of their own homes.
About the Role:
- Plan, implement, and evaluate patient care plans in collaboration with the healthcare team.
- Conduct comprehensive assessments of patients' physical and emotional needs, and develop individualized care plans accordingly.
- Provide education and support to patients and their families to ensure smooth transitions between healthcare settings.
Requirements:
- Associate Degree or Diploma in Nursing required; BSN preferred.
- Current RN licensure in North Carolina or Compact state.
- At least two years of experience in med-surgical, critical care, or home health/hospice/infusion nursing.
Preferred Qualifications:
- Prior experience in Home Health, Hospice, or Infusion nursing.
- Familiarity with OASIS and PDGM protocols.
