We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Signature Healthcare at Home. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to our clients in the comfort of their own homes.
Responsibilities- Provide direct patient care under the supervision of a registered nurse, following a comprehensive assessment and the plan of care.
- Implement current nursing practice, providing accurate and timely documentation of patient services to reflect the plan of care.
- Evaluate patient's response to treatments/medications and provide appropriate pain/symptom management.
- Participate in personal, professional growth and development, and participate in organization's quality assessment performance improvement teams and activities.
- Complete mandatory education requirements annually to maintain competence.
- Attend required staff meetings and patient care coordination activities.
- Perform other duties as assigned by the Registered Nurse or Clinical Manager.
- Graduate of an accredited practical nurse or vocational nursing program.
- One (1) to two (2) years nursing experience preferred, with community health or medical/surgical experience being a plus.
- Currently licensed as an LPN/LVN in the State(s) of planned practice.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Possesses and maintains current CPR certification.
- Knowledge of geriatric population and their common needs and concerns.
