About the Role

We are seeking a dedicated Community Health Worker to join our team at Your Health Organization.

This is a full-time, salary-based position with an estimated annual salary of $55,000 - $65,000. As a Community Health Worker, you will serve as a liaison between patients and health and social services, facilitating access to services and improving the quality-of-service delivery.

Key Responsibilities:

  • Perform home visits, ALF, and ILF visits in designated service areas.
  • Collaborate with care teams and health coaches to provide patient/customer services.
  • Document findings accurately and communicate effectively with stakeholders.
