Job Summary:

We are looking for a passionate and experienced LPN to provide high-quality patient care and support our team in delivering exceptional healthcare services to our veterans.

Responsibilities:
  • Provide prescribed medical treatment and personal care services to patients with diseases and injuries.
  • Collaborate with the core PACT Team and expanded PACT Team to develop patient care plans.
  • Actively assist with patient care, physical examinations, and ancillary tests.
  • Perform annual and routine nursing assessments and preventive health services.
  • Administer medications, apply compresses, and observe patient reactions.
  • Operate diagnostic equipment, collect lab specimens, and submit results to medical providers.
  • Provide patient and family health education, and advise on community resources.
  • Maintain confidentiality of veterans' data and information.
Requirements:
  • Diploma or certificate from an accredited program of practical or vocational nursing.
  • Minimum one-year experience as an LPN/LVN in a related primary care or ambulatory care setting.
  • Credentialed and in good standing through the Veterans Health Administration (VA).
  • Valid, unrestricted license in the state of desired employment.
  • Current certification in Basic Life Support and ACLS.
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.
