Key Responsibilities
  • Provide high-quality patient care in the home setting, including administering medications, wound care, and other medical treatments as prescribed by physicians.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and to address any concerns or questions.
  • Document patient care activities, including assessments, treatments, and progress notes, in a timely and accurate manner.
Requirements
  • Current LVN license in good standing.
  • At least one year of experience in an acute care or equivalent setting.
  • CPR certification with American Heart Association or American Red Cross.
  • Positive attitude and strong communication skills.
  • Dedication to quality patient care and a commitment to delivering exceptional service.
