Job Summary

Your Home Assistant, LLC (YHA) is a leading provider of home care services, seeking a highly skilled and dedicated individual to fill the role of Lead Care Coordinator and Client Service Manager. As a key member of our team, you will be responsible for coordinating the scheduling of caregivers, managing client services, and ensuring the highest level of care and satisfaction for our clients.

Key Responsibilities
  • Orient new caregivers on company policies and procedures, and complete the onboarding process.
  • Arrange and document training for caregivers.
  • Schedule caregivers and notify clients of changes.
  • Perform written assignments of duties on a client-specific basis.
  • Meet with caregivers to educate and provide feedback as necessary.
  • Receive and review referrals of clients and follow up with them as needed.
  • Perform initial assessment visits to client homes, assign caregivers, and complete assessment and open packet forms.
  • Conduct supervisory visits to clients' homes to ensure quality of care and address any concerns.
  • Provide monthly supervision of client care via telephone or other appropriate contacts.
  • Investigate complaints and incidents, and provide counseling to caregivers as needed.
  • Communicate with case managers, physicians, and other providers to ensure effective care.
  • Attend and participate in client care conferences and staff meetings.
  • Maintain confidentiality and adhere to company core values, ethics, and mission.
  • Perform all job functions in compliance with local, state, and federal laws, regulations, and professional standards.
  • Participate in an administrative on-call rotation and assist with client care/shifts as needed.
Requirements
  • High school diploma or equivalent required; associate's or bachelor's degree preferred.
  • Minimum 2 years of experience in home care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Ability to maintain confidentiality and handle sensitive information.
