The Opportunity

We're seeking a skilled Health Coach to join our team at our Denver, CO office, where you'll be working with our Senior Health member population (age 65+). As a key member of our primary care team, you'll play a vital role in supporting patients with their preventive, chronic, and acute care needs. Your primary responsibilities will include establishing trusting relationships with patients and their families, advocating for their needs, and collaborating with our integrated primary care team to facilitate better healthcare outcomes.

Key Responsibilities
  • Capture relevant information about patients' health and healthcare experiences while rooming patients, taking vitals, and performing standard age and condition-appropriate screening assessments.
  • Assist with coordinating post-visit care by scheduling appointments with specialists, coordinating referrals, and sharing information with patients' internal and external care teams.
  • Collaborate with providers to monitor patients' health and ensure they're up to date on preventive measures.
  • Participate in daily operations, including answering phone calls, responding to emails, and ensuring the general upkeep of the clinical space.
  • Provide patient education on chronic disease management and coach patients using an action-planning model based on motivational interviewing techniques.
Requirements
  • At least 1 year of experience in a high-touch customer service or patient-facing role in a healthcare setting.
  • Strong written and verbal communication skills.
  • Experience working on collaborative, diverse, and feedback-driven multi-disciplinary teams.
  • A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges.
  • Proficiency in computer technology, including typing, navigating the internet, and using multiple software systems simultaneously.
