The Transition of Care RN is a critical role at CVS Health, responsible for ensuring seamless transitions for members experiencing significant changes in health status. This position requires a skilled RN to coordinate care, facilitate post-discharge goal attainment, and provide clinical assistance to determine appropriate services and supports.
Key Responsibilities- Complete post-discharge questionnaires and assessments to identify member needs
- Ensure members have filled/received medications and understand how to take them
- Provide benefit education and monitor members for alerts or changes in condition
- Complete inpatient confinement calls and monitoring for discharge
- Manage warm transfers from concierge and engagement hub
- Provide clinical assistance to determine appropriate services and supports
- Evaluate health and social indicators and identify barriers to achieving optimal member health
- Active and unrestricted RN licensure in the state of VA and/or compact license
- 3+ years clinical practice experience as a Registered Nurse
- Preferred qualifications include 2+ years care management, discharge planning, and/or home health care coordination experience
