We are seeking a skilled RN or LPN to fulfill the role of MDS Coordinator. The MDS Coordinator is responsible for overseeing, preparing, and managing the MDS process, which includes developing personalized care plans from the point of admission through to discharge. This role involves directing and allocating essential resources to uphold the company's commitment to ensuring the highest possible quality of life and health for all residents.
Key Qualifications:
- Proven experience in a healthcare environment that provides skilled nursing care, rehabilitation services, assisted living, or residential care.
- Demonstrated ability in formulating individualized care plans.
- Familiarity with standard assessment tools for evaluating resident conditions, including the RAI Manual.
- Proficient in the MDS 3.0 system.
- Understanding of CMS payment and reimbursement methodologies.
- Exceptional observational skills and the capacity to critically assess extensive information.
- Strong organizational skills that facilitate the management of multiple priorities and adherence to an effective schedule.
- Valid RN or LPN license is required.
