We are seeking a compassionate and skilled LPN to provide exceptional care to our residents. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our residents and their families.

Responsibilities:
  • Provide direct nursing care to residents, including assistance with Activities of Daily Living (ADLs) and family/social activities.
  • Identify and report resident medical and behavioral changes, and develop and monitor care plans.
  • Collaborate with other disciplines to ensure comprehensive care and support for residents.
  • Provide daily reports to the Director of Health Services regarding community conditions, personnel performance, and resident needs.
Requirements:
  • Current nursing license valid in the state of Ohio.
  • Willingness to work night shifts and be available for emergencies.
  • Strong communication and interpersonal skills.
