  • Chronic Illness Management: Manage patients with chronic illnesses, co-morbidities, and disabilities to ensure cost-effective and efficient utilization of health benefits.
  • Member History and Care Planning: Obtain a thorough and accurate patient history to develop individualized care plans.
Requirements:
  • Active Texas RN License: Must be currently licensed as a Registered Nurse in the state of Texas.
  • Managed Care Experience: 3-5 years of experience working in a managed care environment with individuals with chronic illnesses or disabilities.
Preferred Qualifications:
  • Service Coordinator Experience: Previous experience as a Service Coordinator or in a similar role is preferred.
