Job Overview

Conifer Park Inc. is actively seeking a Per Diem Psychiatric Nurse Practitioner for our Outpatient Clinic.

Key Responsibilities:

As a Per Diem Nurse Practitioner, your primary role will involve delivering psychiatric assessments and care to patients facing existing or newly identified psychiatric conditions that may impact their substance use disorder treatment.

  • Conduct thorough psychiatric evaluations, establish initial diagnoses, and document findings in patient records.
  • Collaborate with physicians to create practice agreements as necessary.
  • Order and interpret diagnostic tests, including laboratory work, to inform patient care related to psychiatric diagnoses or prescribed treatments.
  • Formulate, execute, and document management plans, including medication and therapy suggestions, while aiding in the continuity of care.
  • Perform follow-up visits, updating diagnoses and medications as required.
  • Monitor and manage medication effects and side effects.
  • Provide guidance and education to patients regarding adherence to prescribed treatment plans and substance use disorder management.
  • Conduct pre-admission assessments for patients exhibiting psychiatric symptoms and perform interviews when necessary.
  • Engage in triage processes within the psychiatry department.
  • Assess and address psychiatric emergencies, including emergency evaluations for suicidal patients, ensuring appropriate care and safety measures.
  • Refer patients requiring more intensive psychiatric care to suitable facilities.
  • Participate in case assignments, multidisciplinary team meetings, educational sessions for staff and patients, and other duties as directed by the Medical Director.
Qualifications:

Applicants should possess a Post Graduate Degree, specifically a Master's in Psychiatric Nursing, along with a valid NYS Nursing License and certification as a Nurse Practitioner in Psychiatry. A minimum of 1 to 3 years of experience in the role is preferred, particularly with individuals dealing with substance use disorders. An active Medicaid Number is highly preferred.
