Responsibilities
The Registered Nurse (RN) Case Manager is responsible for coordinating client care and serves as the primary contact for all healthcare disciplines involved in patient care. This role includes overseeing the frequency of patient visits throughout the care episode.
The RN Case Manager collaborates with physicians and the office to provide updates on patient care.
Key responsibilities include:
- Accurate OASIS data collection
- Maintaining an up-to-date medication profile
- Timely reporting of lab values to the physician
- Participation in weekly case conferences and monthly case manager meetings
Qualifications
Registered Nurses (RNs) must meet the following criteria:
- Current licensure as a Registered Nurse (RN) in the state of employment
- A minimum of one year of clinical experience is preferred
- Demonstrated knowledge and proficiency in current nursing practices
- Possession of a valid state driver's license and automobile liability insurance
