Job Summary

This position serves as the liaison and advocate for individuals with Intellectual and Developmental Disabilities presenting in jails or with criminal justice involvement. The successful candidate will communicate with criminal justice staff to establish a release and effective transition plan back to the community.

Key Responsibilities
  • Develops and implements service and treatment plans to meet client needs consistent with the person-directed planning philosophy
  • Coordinates service provider and program activities, linking and coordinating services available through community resources
  • Provides ongoing targeted case management and serves as a liaison between clients, client families, service providers, and criminal justice staff
  • Identifies clients' needs and service gaps, maintaining case history and progress records and preparing reports in a high-volume case documentation
  • Assists in identifying and seeking resolution to barriers to services for clients, assisting in providing policy and procedure changes and making recommendations
  • Monitors clients' services and evaluates the effectiveness and adequacy of services through site-based visits and collateral telephone calls in a variety of settings
Requirements
  • Prefer knowledge of Intellectual and Developmental Disabilities, Autism, related conditions services, and funding sources
  • Knowledge of community resources, case management delivery systems, principles, objectives, standards, methods, and of program policies and procedures
  • Knowledge of the jail, court, and any other Forensic systems
  • Ability to assess client needs, to oversee and coordinate client services, to monitor program effectiveness, and to communicate effectively
  • Skills in the use of computer and applicable software, ability to prepare and maintain records
  • Extensive travel within Bexar County required, must have reliable transportation, ability to lift 10-15 pounds is required
Preferred Qualifications
  • Prefer experience in case management or social services work
  • Graduation from an accredited four-year college or university with a bachelor's or advanced degree, or an associate degree with major coursework in social, behavioral, human services, or health-related field
  • Prefer ability to meet specific requirements of Title 42, CFR, Section
  • Prefer bilingual fluency in both English and Spanish
