Job Description
This role involves providing evidenced-based care to a panel of patients in collaboration with a dedicated clinical care team. Key responsibilities include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This position reports to the practice manager.

Key Responsibilities:
  • Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
  • Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
  • Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
  • Counsels and educates patients and families on health maintenance and responds to patient care inquiries
  • Evaluates and treats patients' medical needs with safe and high-quality treatment
  • Documents all patient care within an electronic health record in accordance with current policies and procedures
  • Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients

Required Skills and Qualifications:
- Active Advance Practice Registered Nurse, Nurse Practitioner license or Physician Assistant license in good standing
- Current National Board Certification, Family Practice
- A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant in either a family medicine or primary care practice preferred. If not, successful completion of CVS Minute Clinic primary care training program.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions
- Self-motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practice
- Effective verbal, written, and electronic communication skills
- Outstanding organizational skills and ability to multi-task
- Initiative, problem-solving ability, adaptability, and flexibility
- Ability to work remotely in a clinical care team culture
- Ability to work without direct supervision and practice autonomously
- Is proficient with information management and technology
- Willingness to obtain multi-state licensures
