Job Title: Registered Nurse Leader

Job Summary:

The Registered Nurse Leader will support the Center Nurse Executive in delivering efficient and effective nursing care, achieving positive clinical outcomes, and patient/family and employee satisfaction during a designated shift. This role will serve as a clinical operational liaison between the units and the CNE, ADON, and Unit Managers, ensuring compliance with Genesis policies and procedures, practice standards, and regulations.

Responsibilities:
  • Administrative:
    • Collaborate with the Center Nurse Executive to maintain adequate nursing coverage for 24 hours a day, 7 days a week.
    • Effectively utilize existing Client resources, limiting the use of overtime and agency personnel.
    • Participate in staff recruitment, interviewing, and selection of nursing personnel.
    • Assess the work performance of nursing staff, relating to their job description, unit standards of care, and individual goals.
    • Assist with the Individual Performance Improvement Plan for staff whose performance does not meet center expectations, and when necessary, recommend or initiate disciplinary action.
    • Recommend and facilitate employee promotions, transfers, and terminations.
    • Collaborate with other departments to provide timely and effective care consistent with individuals' needs, choices, and preferences.
    • Answer any emergencies throughout the shift, such as changes in condition, starting IVs, or pronouncing death.
    • Foster a strong partnership between the medical staff and the department of nursing.
    • Make recommendations to the Center Nurse Executive regarding nursing care equipment and supplies required to meet patient needs.
    • Promote a culture of safety to ensure a healthy practice and living environment.
    • Participate in meetings to address patient care, survey, and/or standards of care issues as requested.
    • Ensure the building is secure during the assigned shift by checking doors and alarms according to nursing center protocols.
  • Clinical Leadership:
    • Work with the CNE to effectively plan, organize, direct, and implement a comprehensive center-wide nursing program.
    • Provide nursing leadership to meet the goals of the Genesis organization and the individual center.
    • Ensure patient care assignments are consistent with staff competencies, scope of practice, and patient needs.
    • Create an environment that is respectful, team-oriented, and responsive to the concerns of staff, patients, and families.
    • Complete rounds on the units to observe patients and determine if nursing needs are being met.
    • Provide patient care when required.
    • Monitor nursing care to ensure positive clinical outcomes and minimize rehospitalizations.
    • Ensure Point Click Care (PCC) is utilized according to the Business Processes.
    • Encourage communication between licensed nursing staff and CNAs during and between shifts.
    • Encourage shift-to-shift communication between incoming and outgoing nursing staff.
    • Ensure timely and accurate compliance of the MDS process and that it reflects the patient's plan of care.
    • Review prospective admissions in relation to existing nursing capabilities and ensure readiness to treat.
    • Contact attending physicians to obtain orders as indicated.
    • Ensure Physician Orders are followed as prescribed.
    • Oversee medication management to ensure adequate supplies and that all medications are handled in accordance with Genesis policy.
    • Ensure patients' attending physicians and family or responsible party are promptly notified of any significant change in the patient's health condition.
    • Facilitate discharge planning process and provide consultation as needed.
    • Investigate patient/family complaints and take appropriate actions to bring to resolution.
  • Education:
    • Identify educational needs of the staff and communicate needs to the Nurse Practice Educator (NPE).
    • Conduct education in the absence of the NPE.
    • Participate in the evaluation of staff competency.
    • Assist with the orientation of newly hired nursing staff.
    • Demonstrate, teach, and evaluate nursing skills utilized in direct patient care of the unit's specific patient population.
    • Promote professional development and career opportunities for nursing staff.
  • Quality Improvement:
    • Support the CNE in employing a strong Quality Improvement (QI) process for the nursing department.
    • Ensure patients' Care Plans are implemented and periodically reviewed and modified as necessary.
    • Ensure patients' accident/incident, adverse event, and grievances/concerns are fully documented, investigated, reported, and addressed in accordance with Genesis policies and procedures and the Federal/State rules and regulations.

Requirements:

  • Graduate of an accredited school of nursing, college, or university with a current registered nurse licensure by the State Board of Nursing.
  • A minimum of three years full-time or equivalent clinical experience is required, and a minimum of two years of clinical experience in long-term care nursing with one year in a management/administrative or supervisory capacity is preferred.
  • BSN preferred.
  • Maintains current BLS/CPR certification (All SNFs and Behavioral Centers; ALFs/ILFs as required by State).
  • This position requires the ability to read, write, speak, and understand the spoken English language to ensure the safety and well-being of our patients and visitors at the work site when responding to their medical and physical needs.
  • Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy. TSTs will be administered at the work site if required.
