Job Summary

We are seeking a skilled and compassionate Hospice LVN to join our team at Acara Health. As a Hospice LVN, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Administer skilled nursing care to patients of all ages in their places of residence.
  • Participate in the coordination of care with the interdisciplinary team, patient, family, and referring agency.
  • Assist in identifying patient needs and developing care plans in conjunction with the RN.
  • Provide patient and caregiver education on disease management and self-care.
  • Monitor patient progress and adjust care plans as needed.
Requirements
  • Graduate of an accredited school of vocational nursing.
  • Active state license as a licensed vocational/practical nurse.
  • One year of experience as a licensed vocational/practical nurse in a clinical care setting.
  • Reliable transportation and valid auto liability insurance.
