End-of-Life Support Associate

We are seeking an experienced Certified Nursing Assistant (CNA) to join our hospice care team. As a CNA, you will provide high-quality care to patients in their homes, focusing on comfort, dignity, and quality of life. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.

Key Responsibilities
+ Provide direct patient care, including bathing, dressing, and feeding as needed
+ Assist patients with ambulation and transfers, ensuring their safety and comfort at all times
+ Maintain accurate documentation of patient interactions and care provided
+ Collaborate with the healthcare team to ensure seamless transitions of care
Requirements and Qualifications
+ Current CNA certification or equivalent experience
+ 6 months of patient care experience in hospice, home health, or related setting
+ Ability to lift up to 50 pounds, push/pull carts, and stand for long periods
+ Strong communication and interpersonal skills, with ability to work effectively in a fast-paced environment
Compensation and Benefits
+ Salary range: $50,000 - $70,000 per year, based on experience
