Actalent is seeking a Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for providing telephonic triage and care coordination services to our members.
Responsibilities:
- Develop and implement a plan of care based on accurate and comprehensive assessments of the member's needs.
- Adhere to established medical/departmental policies, clinical practice guidelines, community resources, contracting, and community care standards for each case.
- Work with complex cases to promote the delivery of quality, cost-effective health care services based on medical necessity and contractual benefits.
- Provide guidance to the provider network.
- Coordinate follow-ups and referrals.
- Handle 15-30 calls for telephonic information coordination each day.
- Work with Medicaid patients.
Requirements:
- Active RN license in OK.
- Strong written, typing, and verbal communication skills.
- Medical Terminology knowledge.
